COUNTY GOVERNMENT OF HOMA BAY TENDER JANUARY 2023 

The County Government of Homa Bay through the department of Public Health and Medical Services invites eligible bidders for the following project:

Interested eligible bidders may view and obtain detailed tender documents from the following sites from January 25,2023: www.supplier.treasury.go.ke or www.homabay.go.ke or www.tenders.go.ke

Completed tender documents should be submitted through the IFMIS Portal www.supplier.treasury.go.ke on or before February 8,2023 at the time indicated on the portal.

MANDATORY REQUIREMENTS:

1. Copy of certificate of incorporation/Registration;
2. Copy of a valid and current tax compliance certificate;
3. Must submit a copy of relevant current National Construction Authority Practicing License [NCA Category 4 and above in Building Works;
4. Copy of relevant Valid National Construction Authority Registration Certificate fNCA Category 4 and above in Building Works and Mechanical works!:
5. A copy of Current CR12for limited company and for Sole proprietor & Partnership companies to provide copies of directors national identification cards [I.Ds];
6. A valid Single business permit from the relevant County Government for 2023;
7. Dully filled, signed and stamped tender securing declaration form;
8. Dully filled, signed and stamped Self Declaration Form SD2;
9. Dully filled, signed and stamped Certificate of Independent Tender Determination Form;
10. Dully filled, signed and stamped Declaration and Commitment to the code of ethics Form
11. U. Duly filled, signed and stamped form of tender prepared on supplier’s letterhead clearly showing the Tenderer’s complete name and business address;
12. Dully filled Schedules including priced Bill of Quantities;
13. All tenderers to provide Ksh. 4,000,000 tender security in the form of a Bank guarantee from a Financial institution regulated by Central Bank of Kenya [CBKJ;
14. Written declaration letter signed and stamped by an Attorney/ Commissioner for Oaths that the firm /person/ Tenderer is not debarred firm participating in procurement proceedings;
15. Written declaration letter signed and stamped by an Attorney/ Commissioner for Oaths that the firm /person/Tenderer is not guilty of violation of fair employment laws and practices;
16. Tender document MUST be sequentially paginated/serialized/numbered on each page including all the attachments.
