MANDATORY REQUIREMENTS
1. Certificate of registration/incorporation
2. Provide a valid filled up cr12 from the registrar of companies (should be within 12 months and
must be certified) and copies of id for directors
3. Pin registration certificate from KRA
4. Current and valid copy of tax compliance certificate from KRA
5. Current national construction authority (N.C.A) certificate category 6 and above. (both
registration certificate and contractors annual practicing license-builders works)
6. Tender form duly completed, signed and stamped by the tenderer in the prescribed format.
7. Price schedule duly completed, signed and stamped by the tenderer in the format provided
8. Duly filled certificate of independent tender determination
9. Duly filled form SD1 self-declaration.
10. Duly filled form SD2 self-declaration.
11. Duly filled signed and stamped Confidential Business Questionnaire
12. Duly completed and signed Declaration and commitment to the code of ethics
13. Submit an original tender security of Kshs. 100,000 in form of a bank/ insurance guarantee from
approved institutions valid for 154 days from date of tender opening
14. Original and copy of Tender document which MUST be paginated sequentially on all pages
including all attachments.
15. Evidence of having done similar nature, complexity and magnitude of works in reference to
contract Amount-Attach at least (3) completion certificates clearly indicating contract amount
16. Recommendation letters from the respective clients (Accounting officers) of attached completion
certificates
17. Attach the written power of Attorney
- Copy of valid Social Health Authority (SHA) compliance certificate
- Copy of valid NSSF compliance certificate
- Valid single business permit from any county
- Prequalification by NG-CDF Webuye West
- Duly filled pre-site visit certificate
- Original Certification of Tender Documents
- Letter of authority authorizing the procuring entity to seek for confirmation or counter check
any of the information provided in the tender document from a relevant source - Bank statement for the last 6 months not older than one month from advertisement date
(minimum balance KES. 500,000/=) - Any other evaluation criteria as set in the tender document
- Bidders with ongoing works behind schedule will not be considered.
- Public procurement capacity building levy: include a provision for the public procurement
capacity building levy (0.03%), exclusive of applicable taxes.
SITE VISIT
INTERESTED CONTRACTORS ARE ADVISED TO VISIT THE SITES TO FAMILIARIZE THEMSELVES
WITH THE PROPOSED WORKS AND BE FURNISHED WITH SITE VISIT CERTIFICATE.
NB; ALL CERTIFICATIONS MUST BE OF ORIGINAL STAMP FROM COMMISSION OF
OATH, DATED AND PROVIDE HIS PRACTISING LICENCE.

TENDER BOX LOCATED AT MILANI PRIMARY SCHOOL
TENDERS WILL BE RECEIVED ON or BEFORE 26/06/2025 AT 10:00AM LOCAL TIME. TENDERS WILL BE
OPENED IMMEDIATELY THEREAFTER IN THE PRESENCE OF TENDERS AND /OR THEIR
REPRESENTATIVES WHO CHOOSE TO ATTEND. LATE APPLICATIONS WILL NOT BE ACCEPTED.
